The 6-item SF-COG™️ survey measures six aspects of cognitive functioning, including reasoning, concentration and thinking, confusion, memory, attention, and reaction time. In many instances, we recommend taking a multi-method approach that also includes a trained observer’s perspective and/or performance measures when assessing cognitive functioning. The SF-COG™️ is available with a standard four-week recall period, an acute one-week recall period, and with a 24-hour recall period.
