The SF-36 questionnaire is a short-form health survey that measures each of the following eight health domains: physical functioning, role-physical, bodily pain, general health, vitality, social functioning, role-emotional, and mental health. Each health domain score contributes to the Physical Component Summary (PCS) and Mental Component Summary (MCS) scores. As a versatile quality of life questionnaire, SF-36 measures can be derived from SF-36®v2 data, including the SF-6D and SF-6Dv2 preference-based utility indices; a medical expenditure prediction to help understand the economic impact of a patient’s condition and treatment; the Mental Health Inventory Conversion for screening for depression; and a single-item measure of self-evaluated change in general health status compared to one year earlier or one week earlier.
